New Patient Registration

If you are interested in becoming a new patient, please call our billing office at 833.338.4663 to become registered in our electronic medical records system. Once an account has been created for you, we will schedule an initial evaluation appointment with a provider of your choice.

 

New Patients who have already been registered are encouraged to complete our New Patient Packet from our Forms page. Please read it carefully, sign or initial where indicated, and either email to documents@triadpsych.net or bring it with you to your initial appointment in addition to your insurance card.

 

Patients whose information has recently changed are also encouraged to resubmit a new packet with all of their updated information.

If the patient is a minor, please complete our CONSENT TO TREAT A MINOR form. This form must be completed, signed and witnessed before the patient's first appointment to avoid any delay in treatment or rescheduling.

**IF THERE ARE ANY COURT / CUSTODY / GUARDIANSHIP / HEALTHCARE PROXY FORMS, PLEASE BRING THEM TO THE FIRST APPOINTMENT OR THE PATIENT MAY NOT BE SEEN.
